在人體的上下顎骨當中,存在著不同等級的骨頭密度,骨頭也分成緻密骨(compact bone)與海綿骨(cancellous bone),在骨釘的口內操作中,務必要了解骨頭的厚度,才能在骨釘植入後提供較佳的錨定/支抗(anchorage),讓力源能夠穩定的施力,從而使牙齒得到該有的移動

如上圖所示,像IZC與BS(顴骨下脊與頰棚)骨釘均數D3級別,可以不須用motor-driven帶動直接可以鎖入骨釘,像D1則較硬,需先在骨頭鑽孔,再植入
