這是一個20歲左右的成人女性,因為先前騎機車出車禍,整個左邊臉部與牙齒受傷後,上顎的牙齦處出現很多瘢痕組織,經過多年的復原後因為美觀因素前來求診,底下有其全景x光片的圖像,可以看出左臉部受傷嚴重,必需由鈦金屬鋼釘來做固定骨頭結構

左上顎中門牙因為被撞擊嚴重拔除,留下側切牙的轉位,與中切牙的缺失,造成其咬合的景觀非常的不協調

上圖是第 一次黏上矯正器的所見(2008.7.21)

2008.10.02上顎所見,我們把左上中門牙逐步做一個替代性的樹脂牙冠(riding
pontic)來做外觀上的掩飾(esthetic aid ) ,讓治療中的病人對其外觀不至產生遮掩的內向式的感覺
  

從正面觀(frontal view),可以看到左上中門牙暫時冠對外觀上的改善,病人一定會喜歡這樣的

一樣,通過整平排齊的步驟(leveling& aligning),慢慢讓牙弓的形態得到穩定的改善,下顎的2/2因為離主線尚有一段距離,先把其它的牙齒排齊後,視情況加入排列中,若一開始即黏上矯正器,很容易產生咬合干擾與食物殘渣的堆積,對口腔衛生的維護有不良的影響

此案例使用的是日本tomy 公司產的018系統的clippy type 的自鎖式矯正器(TOMY clippy SLB)
